Who we are

Australian Men's Performance and Health Association Inc (AMPHA) is a Queensland incorporated association established March 2026, ABN 32 677 443 647, based in Gold Coast QLD 4220. We aim to handle personal information in a manner consistent with the principles of the Australian Privacy Principles.

What we collect

For general enquiries and partnership or callback requests, we may collect your name, organisation and contact name where relevant, email, phone number, suburb, preferred contact time, organisation type and enquiry content. Please do not provide sensitive health information or identity-document details. We do not ask for them.

Why we collect it

We use the details you provide to respond to enquiries, discuss partnerships, arrange requested callbacks and manage community communications you have requested. We ask for your privacy consent before you submit personal details. We do not use enquiry details for unrelated marketing without permission.

Your self-check answers

The online self-check is for personal reflection only. When you complete it, AMPHA stores only your anonymous yes/no answers and the time — no name, email or other contact details. No score, diagnosis or risk assessment is produced. If you want a callback, you choose to send your details separately through our contact form.

Sharing and referrals

We may share relevant contact details with a partner clinic only after obtaining your separate, specific consent for that referral. Agreeing to this Privacy Policy does not itself consent to a clinic referral. We may receive referral fees. We do not sell personal information. We may disclose information where required by law.

Storage and security

Form submissions are stored in AMPHA's secure online database, which only authorised AMPHA administrators can view. We keep a one-way scrambled (hashed) version of your internet address to prevent spam, never the address itself. If you contact us by email, your message is handled in our email account. We take reasonable steps to protect information from misuse, loss and unauthorised access, and restrict access to those who need it for the enquiry. No internet transmission can be guaranteed secure. Information is retained only as reasonably needed for the enquiry, association administration or legal obligations, then deleted or de-identified where practical.

Service providers and overseas handling

Our email and website service providers may process information outside Australia. We aim to take reasonable steps to protect personal information when using such services. No clinic receives referral details without your specific consent. Contact us if you would like information about how your enquiry will be handled before sending it.

Access, correction and choices

You can request access to the personal information we hold about you, ask us to correct it, or withdraw consent for future use or an uncompleted referral by contacting us. We may need to reasonably verify your identity. Withdrawing consent does not reverse a disclosure already made with your permission or affect records we must keep by law.

Privacy questions and complaints

Email ampha.inc@gmail.com with a privacy question, access or correction request, or complaint. We will acknowledge it and work with you to respond within a reasonable time. If you remain dissatisfied, you can contact the Office of the Australian Information Commissioner (OAIC) for guidance.

Policy updates

We may update this policy as the association’s activities or services change. The current policy will be published on this page. Before live form collection begins, we will update the information about storage and service providers as needed.
